Insurance & Payment

Scheduling an assessment and beginning care

You can schedule a free assessment by calling (954) 431-1681, by completing the Schedule an Assessment form on our website, or by submitting a referral on behalf of a patient. Our team will respond promptly to arrange a visit and discuss the care that best fits the patient's needs.

During the assessment, a member of our clinical team visits the patient at home to understand their needs, discuss their health situation, and help determine the right level of care. We also coordinate with the patient's physician and any necessary orders to build a personalized plan of care.
